There is a higher risk of birth defects if you are older than 35 years, have a family history of birth defects, have a medical condition of your own like diabetes or obesity, or due to the use of certain drugs or medicine. So, as for now you probably have understood the importance of triple or quadruple screening. Let’s get into some details.

What is a triple or quadruple screen test?

A triple or quadruple test is also known as multiple marker test, extended AFP, AFP Plus. It is a basic blood test done between 15 and 20 weeks of the gestation period to detect the level of different substances in your blood: estriol, human chorionic gonadotropin (hCG), alpha-fetoprotein (AFP), and, in case of a quad screen, inhibin-A.

Estriol is estrogen that comes both from your placenta and the baby while HCG and inhibin-A are made entirely by the placenta. AFP is a protein made by your baby and passes in a small amount from the placenta to your blood. These together determine the potential to develop any birth defects. The test determines your risk of having a child with Down’s Syndrome, trisomy 18, and neural tube defects (spina bifida and anencephaly)

The only difference between both is that the quad screen is more apt for identifying the risk of Down’s Syndrome and is more accurate than the triple screen test.

How is it done?

The fluid surrounding your baby in the uterus is collected using a small needle and a sample of cells is withdrawn from your placenta in the tests respectively. A double marker test is one of the key tests to track your child’s health and development in your womb. You could easily get through this test by just giving away a little part of your blood to your doctor.

A double marker test is important as it is a screening test carried out during pregnancy to detect any chromosomal abnormalities in your unborn baby. The test is also useful in finding neurological problems in the fetus like Down’s syndrome or Edward syndrome.

The test is mostly carried out in women above 35 years and who have a family history of birth defects, diabetes, or other complications.

The chromosomal abnormalities can result in severe defects and health problems in the baby after birth. So, it is good to take the test and assure your child’s good health.

The test is simple and non-invasive,  it is carried out by ultrasound and by collecting a blood sample from the mother. The name itself suggests that the test is looking for two markers namely, Beta HCG (Human chorionic gonadotropin) and PAPP (pregnancy-associated plasma protein). Beta HCG is a hormone produced by the placenta of a pregnant woman and PAPP is a plasma protein. If the levels of any of them that is Beta HCG and PAPP are abnormal there might be a chance of chromosomal abnormalities such as Down’s syndrome or trisomy 18 and 13.

It also helps detect any mental disorder, heart disorder, and any other birth-related defects.

Double marker test is also known as dual marker test or maternal serum screening is a part of prenatal screenings in your first trimester which tells you about your baby’s well-being related to genetic abnormalities or neurological complications.

The test indicates the risk of having any chromosomal abnormality or a birth defect such as Down’s syndrome, Trisomy 18, trisomy 13, etc.

Is it necessary to conduct the test?

The double marker test is recommended to every pregnant woman in their first trimester. But it is necessary for women who are above the age of 35 years, or have their previous child with birth defects or have any family history of chromosomal defects or other major complications. So, It’s up to you and your doctor to decide whether you want this test or not.

This is how it is done.

The test simply starts with a collection of a blood sample from your veins and your part is over here.  Now, your doctor examines the blood sample for two markers that are Beta HCG  (human chorionic gonadotrophin) and PAAP (pregnancy-associated plasma protein). These two substances are found only in pregnant women. The test is assisted with an ultrasound called a nuchal translucency (NT) scan carried out before collection of a blood sample, which measures the thickness of space at the back of the fetus’s neck.

How is the result interpreted?

The high or low levels of markers in the blood sample determine the risk for the different conditions. Keep in mind that the test does not confirm the complications. Either you are positive or negative. A positive test result indicates higher chances of disorder and a negative result is in your favor and indicates a lower risk of disorder.

A high level of Beta HCG indicates a risk of trisomy 18 and Down’s syndrome.

A low level of PAAP indicates risk for Down’s syndrome.

But that does not mean your baby is going to have them, the test only indicates the risk, your child still can be fine and healthy.

What happens next?

As a positive result does not confirm the disorder, hence your doctor recommends you some more tests. These are diagnostic tests conducted to confirm the disorder. So do not panic until you know the correct results.
