Judging by the moments in canon Kakashi gets to see ppl reacting to his looks or talking about it and how he seems genuinely confused I think he doesn't really realize how attractive he is, like in this scene for example, when Teuchi and Ayame saw his face
Or in Kakashi Retsuden here, when he heard the maids going on and on about how tall and handsome he is and how he's exactly their type and he was like "I don't understand what they are talking about but whatever!" And I think this is because it's part of Kakashi's personality that he doesn't really accept any kind of praise coming his way which is a result of the things Kakashi went through that made him not really like himself that much and always thinking that he doesn't deserve being praised or admired, like when Tenzo and Yugao call him senpai and say that they look up to him and he flat out tell them "go find a better role model" or when Hagoromo himself was thanking him for his part in saving the world and he was like "um no that was my students, I didn't really do anything, have you seen them? They are so cute, right?" And Hagoromo was like "yeah dude I've seen them but I'm talking about you now not them, you did such a great job too" and Kakashi was like "oh, no no no, you must have confused me with my friend Obito, it was all him, I did nothing" and I love how Hagoromo knows that too and insisted to get it through Kakashi's head that "yeah I know what your students and Obito did but I'm talking about you now not them, you did amazing and they wouldn't have succeeded without You, now deal with it!" Lol so yeah Kakashi just doesn't accept any kind of praise in general, he doesn't like to admit that he is good at anything at all, the only few times he admitted that he was good at something were only for the benefit of his students when he tried to calm them down and tell them to not worry, he got this, he will protect them.
