Peritoneal pseudotumoral actinomycosis: A case report
Actinomycosis is a suppurative bacterial infection caused by Actinomyces species. The majority of infections in humans are related to Actinomyces israelii. Abdomen is the third body site for the infection of actinomycosis (20%). If the circumstance of discovery is an abdominal mass, the local lesion needs to be differentiated from abdominal malignant neoplasia. The clinical and radiologic findings of actinomycosis are variable. Diagnosis preoperatively is rarely made due to variable clinical presentations and confusion often with tumor origin. The majority of cases are diagnosed after surgical intervention with resection of the lesion and histological examination. A 61-years-old man, without medical or surgical history, was admitted complaining of left abdominal pain for three months without bowel dysfunction. Physical examination showed a good general condition and no fever. Abdominal exam objectified a large tinder mass of the left upper quadrant fixed to the abdominal paw.
This 60-year-old woman complained of swelling of the medial lower eyelid with discharge. Examination revealed inflammation of the lower canliculus with pus. The canal oculus was incised and curretaged multiple yellow concretions were expressed. The canaliculus was irrigated with antibiotic and treated with iodine application. Actinomycosis of the face
While common in cattle (especially young weaned bulls and heifers), actinomycosis is rare in humans. It's an opportunistic infection, becoming entrenched while the immune system is compromised by disease or malnutrition, and when it becomes established inside the body (most often the chest) is often misdiagnosed as a neoplasm (neoplasm = "new growth" - a tumor).
On the face, it causes a slow-growing, lumpy surface, that ignores tissue layers, and creates sinuses (holes) that spontaneously heal and recur as the disease progresses.
While once thought to be a mycosis, or fungal infection (hence its name), actinomycosis is now known to be caused by anaerobic, gram-positive bacteria. As it's still a very uncommon condition in humans, actinomycosis has not developed much resistance to antibiotics, and is generally responsive to penicillin and amoxicillin. However, because the bacteria can become so entrenched in areas that do not receive high amounts of blood flow, the antibiotics must be continued for up to a year, if the disease has been present for a long time.
