When Natasha mentions her ledger, she likely means an actual record she kept. Assassinations are a business, after all, and even KGB operatives keep accurate records.
Maria Hill's ledger is, and a fine point of distinction here should be made because Natasha's consists of everyone she personally killed while Maria's is the death of anyone/everyone she felt responsible for causing, mostly in her mind. Faces, names, sometimes the flash of a trait or favorite food. She has no idea how similar Romanova is in remembering their dead.
But Maria was a soldier, trained to protect. She was an international copper, and a commander. Natasha worked, for the most part, alone, and she had been made to kill. It was a different approach to the issue at hand, Maria felt.
She'd always feel more guilt, but Natasha truly mourned. Somehow, Maria Hill thought that made the Black Widow the better person...
