Frank Hayes was a jockey who had never won a race, alive at least. He had a heart attack mid-race, but his horse "Sweet Kiss" went on to win the race. She was nicknamed "Sweet Kiss of Death" afterwards.
Back in the day, the gear of being buried alive was so common that safety bells would be installed to make sure it wouldn't happen. It's believed that we got the phrase "saved by the bell" from this, though I have heard other theories.
