Not sure when you posted this, I only just saw. Did you start the mission before the citadel attack? Quite a few of my missions screwed up -
I had the mission before the Citadel attack, but I didn't do it until afterwards (I think so anyway because I did the mission a few weeks ago but it still says I never).
