OK I've been away since the episode aired but am I really the only one worried here?

Are they implying something with the whole drinking-a-bottle-a-day thing? Because I felt that Pear's dad was very reluctant and (weirdly?) uncomfortable here, and then at the hospital, at the idea of treating Kawi's dad... 😕
