Hey Sleepy! I'm guessing you mean a lethal disease too. Sad scenario, potentially triggering for people who do not want to read about long terminal diseases, so I'll put it under the cut.
I think in every scenario it would take a while for Kid, the crew and (possibly) the children to accept it and give up. They would travel the whole Grand Line up and down, even the four Blues if necessary, in search for any kind of cure, doctor or devil fruit that might help birdie.
Depending on when this was happening, Trafalgar Law, Chopper, Marco, and other doctors would all be involved for their expertise at some point.
If nothing could be done and birdie's conditions deteriorated too much, I guess Killer would somehow convince Kid to stop the desperate spin around the world and just remain with her for whatever time is left. If it was during or after Emperors, their children would be present as well.
It would be hard for the crew, especially Heat, but they'd eventually accept it and just give in to the sadness. Same for Eustass Law, he'd break down in private but be very practical while around others. Killer would go in complete managing mode and block out everything until after birdie's death (with Reiju's support if she were already in the picture). Kid and Doruya would be the difficult ones, because I figure they wouldn't be able to accept the situation until the very end, and then just freak, letting rage and destruction devour them.
Honestly, if it's an Emperors timeline, the presence of the children might save Kid. Birdie would make him promise her that he wouldn't let her death stop him from being a good father, and Killer would make everything he can to help him. This could eventually serve as the tether that brings Kid back to life.
I think Law would be the best one to manage Doruya, with a mixture of fighting, talking, and letting her erupts when she needs to.
As for Kuzan, he's so young that he'd be shielded from a lot of it. Birdie would make sure that he spends a lot of time with her for as long as possible. Finally they would tell him, he'd have a crisis, but he's an all-out child, and that would help in the long run. His care would be delegated to the crew a lot, I can see Heat and Amina taking over for a while, especially in birdie's final days and the period immediately after.

The thing is that the Stampede special story (link) is not canon; it doesn't belong to the Captiveverse, but you can imagine it being part of the Emperors storyline (why not?).
Birdie definitely would have used it. I imagine she kept it hidden for a while until she finally mustered the guts to put it on. She concealed it beneath her normal clothes, which was not comfortable. And when Kid inevitably tore her clothes apart, he was met with a surprise.
That became a looong night.
I mean, the first round was pretty hasty because Kid had trouble restraining himself. But the second round, oh boy. Then he took his time.
After that, he kinda forced her to wear it underneath her regular attire for a week.

I've already mentioned something here, and if you've read Emperors, you know that there have been conflicts on handling the children (more on this in the upcoming chapter of Emperors). But I'll tell you something new here, because I don't know if I'm going to incorporate it anywhere in the stories.
When the twins were around one-year-old, a little pirate alliance defied the Red Emperors by attacking islands under his name. It was the first time that Kid left birdie alone with the children on their main island, thinking that now they were old enough for her to handle by herself (and some other members of the crew). Birdie was not too happy about the situation, she wanted to send Killer to deal with the enemies, but eventually she realized how restless Kid was and gave her blessing as he promised he would be right back anyway.
Unfortunately, well... once at sea and in battle mode, Kid got a liiiittle carried away. Once they sunk whomever they needed to sink, Killer warned him that they should head back, but Kid pushed to patrol around, attack more ships that "were bordering too close" to his territories, and maybe conquer one more island. This spree lasted three days, then he realized that: a) it wasn't the same without birdie, b) he missed her and the squirts, c) he fucked up.
He tried to call her that evening, but she did not pick up. His anxiety turned into a fucking twist in his guts and he woke everyone up to head home at once. When he arrived (more than a day later), birdie did not greet him, did not speak to him, and dumped children duties on him for two days. Kid was panicked even more because he didn't even have the time to grovel properly, with the twins to handle. He was such a mess that the crew was frozen by the situation.
After a couple of days, birdie cooled off and accepted Kid's continuous attempts to get close. She said he was forgiven, they made out, and just when Kid was relaxing, she said there wouldn't be sex for the next two weeks.
So this is the legend of the Two Weeks of Hell on the Red Emperor's island.

I think running away and disappearing for days (on the island) to pursue some adventure is already one thing. Doruya used to do it all the time even at a young age, but if Sora gets dragged along Kuzan for this ordeal, she’ll be stressing the whole time.
I imagine when they come back and they are scolded, Sora just takes it heads down, feeling mortified, whereas Kuzan doesn’t even listen, he just stands there with a bright smile thinking about his next adventure.
Doruya and Law are too old to hang out with Sora the same way. By the time she’s old enough for such things, they won’t probably be on the island any more.
The parenting style is also very different. I can see Killer super stressed as soon as Sora doesn’t come home for dinner, going around looking for her, and when he rushes to Kid and birdie, they look at him like
“Yeah? So what’s the problem?”
“Where’s Kuzan?? I need to ask him!”
“Oh, right, he’s not here either. Good, they’re probably together then.”
“PROBABLY???”
Poor Killler is gonna have a hard time! 😭 Reiju won’t likely stress as much.
Sai and Baby 5's daughter must be a menace from what Sai implied in Emperors (born in a powerful family, high self-esteem, strong...a walking disaster child literally)

Though the couple's marriage was rushed, I liked Sai and Baby 5 together. Sai is a good man, and Baby 5 needs someone who would respect her.

This is actually a good question. I guess they ended up talking to each other when they both were taking a somewhat guarding stance for their own leader. You know, as each of them is the responsible one of their bunch, they often fulfill that role.
They probably connected through it without spending too many words. Reiju can be pretty flirty, so she might have initiated/instigated the physical interaction, with Killer just taking her up on it, since there is chemistry there. It was a quickie, intended to remain isolated and casual.
