Went to the clinic for my Clozapine blood test and my skin was melting off and I saw a dead body on the floor. Didn’t say anything.
I'm now on injectable weightloss meds.
Cheers Clozapine for fucking my physical health up so much
Kind of want to skip my Clozapine tonight…
Well, I had my last Clozapine blood test of the year.
Next week is the first one of the year.
I’ll be so happy when these blood tests are every two weeks or even once a month.
Been on clozapine for 2 years for paranoid schizophrenia, it’s been going well. Still have psychotic symptoms but they have been much easier to deal with.
Until the last few days… things feel really difficult. The voices content is changing and I’m feeling ~yeet-y. (Am safe tho) I wonder if my levels are perhaps low, or maybe I’m just broken beyond repair.
Rethinking Clozapine Monitoring
New Video: Rethinking Clozapine Monitoring
Clozapine remains psychiatry’s most effective antipsychotic—but its use is often limited by fear and confusion around blood monitoring.
The truth is this: 🩸 Agranulocytosis risk peaks during the first six months and then drops substantially—but never reaches zero. 📉 The FDA recently rescinded the REMS, but left the package insert requirements intact, leaving clinicians uncertain about best practices.
In this video, I review the new pragmatic monitoring recommendations
Weekly ANCs for 18 weeks, then
Monthly until two years, and
Consider discontinuing routine monitoring after two years if no abnormalities.

The key is shared decision-making—document your rationale and educate patients about early warning signs like fever or sore throat. And don’t forget: an annual CBC remains essential for catching smoldering dyscrasias.
🎥 Watch now to learn how to balance safety, practicality, and patient autonomy in modern clozapine care.
Be better than me, gang. Refill your meds before they run out. Don't procrastinate, do it early. You might think a day or two won't make a difference. You would be wrong.
Signed,
Out of clozapine and wide awake
