I think it's primarily a cost issue. Most elections are run by counties with only a few states that bear the complete cost. It's more cost-effective to use older equipment, less and untrained staff, and to only do limited voting locations. The Help America Vote Act of 2002 did force all states to get electronic voting equipment but the federal gov't bore most of that cost.
Okay. Up here, federal elections are funded and run by Elections Canada, a non-partisan organization that receives it’s funding from the feds. They cover everything from educating and paying staff to printing ballots.

That doesn’t make sense to me. I’ve worked for Elections Canada quite a few times and the standardization means that we have very clear training and procedures and a long list of checks and balances that ensure voter fraud is not a thing that happens.
