so the dsm5 - the diagnostic and statistical manual of mental disorders, that is - says about gender dysphoria:

for a person to be diagnosed with gender dysphoria, there must be a marked difference between the individual's expressed/experienced gender and the genders others would assign him/her, and it must continue for at least six months. in children, the desire to be of the other gender must be present and verbalized.

in volume 9, issue 3 of the medical journal the journal of sexual medicine, an article on pages 751-757 was published in march 2012 that says the etiology (cause) of gender identity disorder remains unknown but their findings suggest genetic factors may have a role.

the dsm5 also says that:

when it comes to access to care, many of the treatment options for this condition include counseling, cross-sex hormones, gender reassignment surgery, and social and legal transition to the desired gender. to get insurance coverage for the medical treatments, individuals need a diagnosis.
