How can I make my cousin understand that he's offensive?

So I don't know how many people are gonna view this or if I'm actually going to get a response but here's the thing: I don't know if he's like this just with me (although I've seen him being like this with some of my friends and online) but he's very offensive, from the dirty talk to innuendo that aren't innuendo because he's like plain out saying it (I don't know to describe it but imagine you're typical person judging and disrespectful person, the one you always find in your neighborhood, that's him) it even got to the point were he even insulted me because I've got diabetes. And name calling and rude jokes aside he is very narrow minded. I don't want to change him, because honestly I don't give a damn, I just want him to understand how rude and disrespectful and sometimes disgusting that is, and to let him understand to be different, at least when around me. I mean he's not incapable, he has critical thinking even if he acts like a child. I just hope that's even possible, because when you're from an Italian background cousins and family alike are inevitable and often
