Of course my newly admitted UGIB and ETOH withdrawal patient had to have a BM at shift change. So we walked the unsteady motherfucker to the bathroom where he proceeded to do his business... including peeing ALL OVER THE FLOOR. Then he tried to clean himself up independently and was only successful in getting melena ALL OVER HIS HANDS and under his fingernails.
My coworkers are yelling across the unit after I finally got my s/p crani, extremely likely to get delirious patient to sleep...

Running CRRT on a futile case tonight. Patient with metastatic cancer, liver, and kidney failure was sent home on hospice by another hospital the day before they came to us. Spouse found them down (in shock) and called EMS asking to be brought here. I think they’re hoping for a miracle. It’s not looking likely. The cancer didn’t respond to treatment and now we’re just prolonging their suffering.
Patient is a full code. 🙁 I get that they’re young and the family isn’t ready, but this will not have a good outcome. I don’t want to code them. Right now the plan is to keep doing what we’re doing, but we won’t escalate care, i.e. we’re not going to add any more pressors. So why are they a full code? Just want them to die violently? Is this really what the patient would have wanted? I guess we won’t ever know.
It’s cases like this that make me want to get the hell away from adults ASAP.
I was just trying to mentally prepare myself for what I was going to walk into tonight and my friend had to play me like that... rude.
