Tumor Immunotherapy Directed at PD-1
Different anti-cancer strategies attempting to interfere with immunresponses have been tested. Cancer takes advantage of the ability to hide from the immune system by blocking T-cell activation. Blockade of cytotoxic T-lymphocyte–associated antigen 4 (CTLA-4) by antibodies (ipilimumab or tremelimumab), and therefore enhancing T-cell activation provided the first evidence of improvement in overall survival for the treatment of patients with metastatic melanoma. Due to unregulated, generalised T-cell activation in the lypmh node (priming phase), clinically significant inflammatory or autoimmune toxic effects are seen in 20 to 30% of patients.
A new approach to counteract immune system suppression by targeting programmed death 1 (PD-1) receptor, another inhibitory T-cell receptor that is engaged by its ligands within the tumor microenvironment, was tested in two clinical trials. The increased selectivity for immune suppressive signals that are delivered directly by the cancer, together with the role of PD-1 in regulating predominantly the effector phase of T-cell responses, predicts that PD-1 inhibition will have fewer side effects and greater antitumor activity than CTLA-4 inhibition.
