So it seems that as time passes, John's vision of Mary depends on what he's seen or thought of recently.
Friendly reminder that Dead!Mary is John's subconscious..... it's all in his head... everything Mary says... it's John saying it... just like in The Abominable Bride, it was in Sherlock's head...
Sherlock's Dead!Mary
So... I'm a really big fan of Dead!Mary. She keeps on advising John to do right by Sherlock. "Christ, John, Stay... talk!" "Go and solve a crime together. Make him wear the hat!" "John, do better." But I think it's possible that we're also seeing Sherlock's Dead!Mary. When John is about to walk out of 221B, Dead!Mary tilts her head as if she's nudging John to stay... OR as if she's waiting for Sherlock to do the right thing. And Sherlock says, "Are you OK?" And that's plus-level not like Sherlock. He doesn't do that - not about feelings, anyway. THEY don't do this sort of thing. Sherlock puts the hat on, too. Which leads me to believe he's hearing Mary - either in the flat here or maybe at the hospital? In conclusion: Dead!Mary: the best couples counselor ever.

Mary got the taxi like she always did to the school. However today was different the driver skipped the light and the two buses hit the front and the back of the cab, sending it spinning across the road, Mary's ribs where crushed and she passed out. She was rushed to hospital along with the taxi driver, he wasn't as badly injured as Mary. Mary had a punctured lung and breathing was difficult, she lay there looking up at the ceiling, muffled voices of doctors, she lay there with IV's and a oxygen mask, tears fell from her eyes. She didn't understand what was happening, her shallow breathes burning her throat and her heart was pounding trying to pump around enough blood. "I doubt she'll survive, we have to keep her comfortable" She heard the doctor say to someone, someone she couldn't see for they were at the door.
