The Delta Flyer from Star Trek: Voyager.
Voyager season 5 episode 6 might feature the single worst decision Janeway ever makes.
They're celebrating the installation of a "quantum slipstream" engine which they seem to believe will be the magic bullet that will get them back to earth in 5 minutes. Shockingly it's Tom Paris who is being the rational one and says "this thing is untested and I have serious concerns about how safe it is to use"
He and Harry run numerous simulations using the slipstream drive and they say "23 simulations and 23 catastrophes" so this slipstream engine has a 100% failure rate, and yet they were throwing fucking confetti around engineering to celebrate
When given this information, Janeway still orders them to go ahead with the planned flight using the new engine. Chakotay tells her it's a pretty big risk and she's basically like "yeah well I'm bored. We've been in the delta quadrant for long enough and I'm willing to take this chance.
In doing so she is responsible for the deaths of literally everyone on board the ship.
In the "future" sections of the episode we see an older grizzled Harry Kim who is obsessively wracked with guilt, blaming himself for the loss of Voyager and the deaths of its crew but like, it was Janeway who gave the orders. Like yeah Harry was very certain about his stupid plan of using the delta flyer to navigate ahead of the ship or whatever but like, why not run tests and simulations to see if that worked either? She was ready to launch that shit before Tom ever ran any tests on it too. It's just astoundingly bad judgement even for her.
"Drive" (S07E02, Stardate 54058.6) has a lot going for it with the race, the relationship growth for Tom and B'Elanna, the mystery of the saboteur and Voyager's commitment to peace. But B'Elanna's "bad match" thoughts are a weak plot point and we really missed out on a Voyager wedding ceremony.
