Mostly through action, inaction and physicality. Pyramid Head speaks no discernable language to human ears... but I like to imagine with certain monsters (such as Valtiel) and Alessa there is some kind of telepathic connection that can verbalise thoughts and conversation. Maybe there is even some kind of talisman or token that allows regular humans to hear him in the same way! To mortal ears, however, the only voice you will hear from him is an animalistic murmur, a sound synonymous with the rumbling of contained, carnal beasts... Otherwise, he can only be interpreted by others through physical expression... for those that are guilty, that expression will be especially violent and antagonistic - mirroring their own atrocities with his characteristic lumbering brutality. For those that are innocent, he seems ‘dormant’; surveying them or ambling around Silent Hill with no indication of pursuing them... especially on his days off... The only way he conveys any kind of non-violent disposition, or complete lack of intention to judge and persecute, is by leaving his great knife impaled in the ground. This is likely the case around children and Alessa. Regardless of who he meets in his local area, however, he is empathic with their suffering - indicated by when he grips his helmet in an attempt to tear it off. He CAN also write! When not patrolling the town for guests, he is carving holy icons and symbols into the earth with his blade, these illustrations probably only perceptible in their pattern and complete image from the viewpoint of a Google Maps drone some fifty feet high... He is prone to this behaviour in DBD trials too, but mainly when Cheryl is present. Sometimes his crop circles are cut off by a map’s significantly reduced land size and he flails at the walls inhibiting his creative expression of his devotion to God... If you manage to successfully connect with Pyramid Head on a telepathic level, he still doesn’t have any clear-spoken voice. What he ‘says’ simply manifests in your consciousness, much like someone’s ‘head voice’ when reading a book, where the interpretation of their native words and syllables feels inexplicably innate. For those that are guilty of sin, the internal sonicality is abrasive and otherworldly - like this (epilepsy warning) - with the potential to cause pain. For those that are sinless, the noise is much less of an assault on the senses but is still capable of evoking the same unnerving sensation as an echo of a voice trapped in radio static... basically these 20 seconds of Rain of Brass Petals.
