In Caesar’s Revenge there aren’t any health pickups - instead the player must recover by chaining kills and entering bloodlust. There is also the moral decision of whether or not the player chooses attack civilians to regain health. Caesar himself fought for the common people and even after death he had many supporters. What is the price of vengeance?
It appears that "AAA" game devs are still convinced that "only boys play shooter games" and "boys will only play a game with a female protagonist if the perspective is third-person so you can see her butt." On the other hand, there are a bunch of indie FPS games where you play as a woman.
