S1-4: Realms of Horror, 1987 supermodule with Larry Elmore cover, featuring Acererak and runes from S1: Tomb of Horrors, the titular volcano from S2: White Plume Mountain, an android from S3: Expedition to the Barrier Peaks, and the vampire Drelnza from S4: The Lost Caverns of Tsojcanth (who normally wears heavy armor). The dragon looks like Jeff Easley’s dracolisk from Tsojcanth. The only true dragon in those 4 adventures is the blue dragon in Tsojcanth, but the name of the game says dragons so might as well put one on the cover.
Beneath the wondrous lanthorn lies Drelnza, daughter of Iggwilv (Jeff Easley, AD&D module S4: The Lost Caverns of Tsojcanth by Gary Gygax, TSR, 1982)
