IIRC it was a prank by one of the senior developers who tricked a 3D artist on the team to make a dick model, then went hah syke, we can't ship explicit content in this game! So they had to cut the dick model out, which is why it's just a black hole instead of the standard smooth Ken bottom everyone else has.
After they shared that story, a fanartist did a render, and the developers saw it and said it was very accurate.
Or maybe there's a mod restoring the *ahem* cut content. I dunno.
