i think there's more of a gradient between the kind of ear worm / echolalia repetition that some people have and intrusive thoughts than is generally thought, at least as someone who experiences both of those
oh huh, i don’t have experience with echolalia but that makes sense. having a brain stuck on certain thoughts and unable to pull away...could be a similar mechanism
