Developed by a team under Assistant Professor Tetsuaki Baba at the Tokyo Metropolitan University and shown at SIGGRAPH earlier this year, Gocen is a handheld scanner that can read sheet music and play it back in real time. As it also recognizes text that specify instrumentation, I wouldn't be surprised if the technology eventually progresses to read and perform a full orchestral score.

Researchers at Tokyo Metropolitan University have created a system that is able to "read" musical notes, either printed or hand written, and translate that into music. It's a cool bit of optical character recognition and a neat interface that could be a fun new tool for interactive media.
