AP and lateral x-rays of both ankles, images on the right half reveal a large posteromedial Thurstan-Holland fragment on the distal tibial growth plate AO 43t-E/2.1 [Salter-Harris II] and subtle epiphysiolysis of the fibula AO 43f-E/1.1 [Salter-Harris I]. Both collectively comprise a Dias-Tachdjian grade II supination-inversion injury variant (A). Coronal and sagittal CT cuts show growth plate is intact (B). Intraoperative fluoroscopy shows reduction of the fracture with provisional pin and cannulated screw fixation, postoperative radiography (C). Follow-up CT cuts show fracture line resolution (D). Five-year visit after screw removal shows excellent healing with implant track, and expected epiphyseal scar formation (E).
