In cardiac arrest scenario especially due to heart attack, would the patient always goes throught v-fib or whtever non-flatline pulseless rhytm, or the patient can go from normal rhytm straight to flatline?
For a heart attack, or myocardial infarction, I would expect the patient to go into an abnormal rhythm first. During a heart attack, lack of blood flow to the heart muscle (myocardium, which makes up the ventricles) causes it to die, which presents as EKG changes, most notably ST-elevation, also known as "tombstones." (see below)
As more ventricular tissue dies, the patient will likely go into a terminal ventricular arrhythmia like ventricular tachycardia or ventricular fibrillation. Unless the the entirety of both ventricles were knocked off all at once, the patient wouldn't suddenly go from sinus rhythm to flatline.
