Examination: Call was made to police by Octavia Greene who reported that the patient was jerking (myoclonus) and convulsing before having fallen completely unconscious. An ambulance was dispatched directly to the residence of 48 Walton Cres. Oxford.
The patient was reported unconscious by paramedics on arrival at residence and remained in that state when admitted to hospital via the emergency room. The patient was and still is completely unresponsive to sound, movement and any other external stimulators. There was physical evidence of cyanosis present as a blue tinge around the lips, mouth and fingertips of the patient.
Scans found a toxic and highly concentrated mixture of the opiode diamorphine (Heroin), alcohol and the benzodiazepine anxiolytic drug alprazolam (Xanax) in the patient’s system presumably having been there for just under two hours. Diagnosis: Taking into all prior information collected during examination it is likely the patient has a toxic hypoxia-anoxic brain injury which resulted in comatose state. Actions taken: Emergency paramedics’ monitored cardiac activity provided supplemental oxygen and airway support on route to hospital to try to mitigate brain damage. On arrival, patient was then connected to a ventilator while Naloxone (NarcanTM) was administered to counteract opiode overdose. Glucose levels were checked and (IV) fluids were administered. Scans were administered and final diagnosis was made. POA: Move to ICU. Preform tracheotomy and placement of catheter. Send for MRI’s to assess extent and severity of brain damage. Monitor and respond as per procedure for comatose patients. Contact family immediately.
