Several weeks ago we were consulted on a patient with flu. His case was pretty nasty. Not only did he land himself an ICU bed, he was started on ECMO shortly after getting to the hospital.

I was on ID at the time and we were consulted because the intensive care unit docs were concerned that he wasn’t able to absorb Tamiflu due to poor gut perfusion while on ECMO, so they wanted an IV equivalent. A med so hard to get it can only be written for by an ID doc and wasn’t even available in this state. Luckily we were able to track it down and thought maybe we’d have a chance of getting on top of this thing.

This morning I found out that he developed a massive pericardial effusion. The cardiologist on call pulled 50CCs of dark red blood off his heart. Several hours later it started to accumulate again.

His parents decided it’s was time to let him go.

I stand outside his room, haunted. He had epilepsy and ADHD, but otherwise was a pretty normal kid.
