🧠 Key Insight This case report describes a 32-year-old female presenting with pain and swelling over the dorsum of the foot, where imaging revealed a multiloculated cystic lesion in the talus suggestive of an aneurysmal bone cyst (ABC). Diagnosis was confirmed through biopsy and immunohistochemistry, differentiating it from other bone tumors. Instead of conventional surgery, the patient was treated with percutaneous polidocanol sclerotherapy, a minimally invasive technique that promotes lesion healing. The patient showed significant symptomatic relief within 1 month, and with repeated injections, achieved progressive sclerosis and near-complete resolution at 2-year follow-up. This case highlights that ABC can occur in rare sites like the talus and can be effectively managed with non-surgical treatment options, avoiding complications of open procedures

🧠 Key Insight This case report presents a 63-year-old woman with a massive recurrent atypical lipomatous tumor of the thigh after three previous surgeries over 8 years. Imaging revealed a large tumor involving multiple thigh compartments and surrounding the femur and femoral vessels. Surgeons performed wide tumor excision with reconstruction of the femoral vessels using a PTFE graft, followed by prophylactic intramedullary nailing to prevent fracture after planned radiotherapy. Final histopathology revealed liposarcoma with areas of low-grade dedifferentiation (FNCLCC Grade 3). The patient received post-operative radiotherapy, and at 35-month follow-up she remained disease-free and able to perform daily activities independently, highlighting the importance of adequate surgical margins and multidisciplinary management in soft-tissue sarcomas.

🧠 Key Insight This case report describes a 33-year-old male who presented with low back pain and bilateral radiculopathy due to an L5–S1 lumbar disc herniation along with a left paraspinal soft-tissue mass. After conservative treatment failed, the patient underwent endoscopic discectomy and initially experienced significant pain relief. However, once non-steroidal anti-inflammatory drugs were discontinued, he developed recurrent pain radiating to the thigh. Repeat imaging showed no recurrent disc herniation but persistent paraspinal mass, which was surgically excised. Histopathology confirmed a benign schwannoma. The case highlights that schwannomas possess an inflammatory tumor microenvironment that can mimic symptoms of disc herniation and may temporarily respond to anti-inflammatory therapy, potentially masking the underlying tumor. Surgical excision ultimately resulted in complete symptom relief
