🧠 Key Insight This case report describes a 32-year-old female presenting with pain and swelling over the dorsum of the foot, where imaging revealed a multiloculated cystic lesion in the talus suggestive of an aneurysmal bone cyst (ABC). Diagnosis was confirmed through biopsy and immunohistochemistry, differentiating it from other bone tumors. Instead of conventional surgery, the patient was treated with percutaneous polidocanol sclerotherapy, a minimally invasive technique that promotes lesion healing. The patient showed significant symptomatic relief within 1 month, and with repeated injections, achieved progressive sclerosis and near-complete resolution at 2-year follow-up. This case highlights that ABC can occur in rare sites like the talus and can be effectively managed with non-surgical treatment options, avoiding complications of open procedures
