Given all the additions they gave to ukitake's history in the last episodes, if it ever came to tybw happening on your AUs would Kaien know about mimihagi and the kamikake ritual? Would he stop it? Would he be up there with Ichigo?

Presuming I choose to keep TYBW at all, my answer would depend on the universe.
I would say generally, yes, Kaien does know. But the timing of how he got to that information changes.
In Black and Blue verse, I imagine it was a secret Ukitake confides in Kaien. I do have drabbles written about BB!Kaien's reaction to Ukitake's sacrifice. I held off on posting them because they would contain spoilers for Kaien's powerset, with those same spoilers being the reason why Kaien is physically unable to go to Reio's palace (I might just post them anyway with the spoilers omitted).
In OMTSYA, presuming my events still loosely followed canon, I think Ukitake would still confide in him, but the timing would be different. I will also preface by stating Kaien and Ichigo already hate Yhwach (and by extension the Quincy as a concept) with a burning passion, and as the same hybrid as Ichigo, Kaien would be in that Reio palace fight. When Kamikake initiates, Kaien would be up in Reio's throne room. For forcing his beloved ex-captain-slash-work-dad to sacrifice himself, Kaien's hatred for Yhwach would transcend from "this guy needs to die yesterday" to "I will bathe in you blood, tap dance on your entrails and erase your stain from existence".
The latter is if I choose to keep the Mimihagi stuff, of course. I'll be honest and say I never really liked it. Like much of that arc, that plot point felt shoehorned, like Kubo thought "oh shit, he's literally the perfect counter to all Quincy power. He's gotta die. But I need to make him artificially relevant first!" and pulled this out of his behind.
Plus, I have this cute idea of Kaien and Ukitake bonding over their respective sicknesses (before Kaien gets his memories back and the underlying issue is resolved, obviously) and Kaien offering to introduce Ukitake to a cardiologist in the human world when the Rukia execution mess is over. Then a comedic omake of Ukitake showing up on the Kurosaki clinic doorstep to take him up on it.

Behind the name
So I was doing my own research because all this Mimihagi-sama shit has me hyped up so I looked into some Japanese webpages about manga, specifically Bleach and found this about Mimihagi-sama:
NOTHING!
So far the name has been written in Katakana like this ミミハギ様 wherever in the internet I go. Katakana are the Japanese characters for writing foreign words or just sound. Now I'm not sure whether that name is foreign or not, I don't think it is, so that means that there is no meaning behind the name, just the sound.
The last character you see on the name is a Kanji but it's just "sama" a honorific.
So until someone who knows more about this than I do comes up with a better explanation this is all we have so far.
I think we overlooked something. Ukitake's incantation for Kamikake goes:
Mimihagi-sama… Mimihagi-sama… Please lend me your powers…
With the power you have granted me I shall release it…
What exactly is he going to release? It couldn't be Mimihagi-sama, it's already out. My theory is that Ukitake's Bankai is so strong that he needs the power of a God alongside with his to release it because his power along and in the condition he was it would've been improbable. That or this power he is going to release is completely unrelated to Bankai.
ok so I have to admit that the most shocking part of today's chapter was seeing Uki as an ill baby (yes I did read the last two pages, that was shocking too but I kinda saw it coming), laying there, suffering like that. it broke my heart and tears came to my eyes. think about it, it probably took him some years until his lungs were ok to work. he probably didn't have a childhood at all, he was not allowed to go play outside with his other friends and probably had to take a lot of medicines. he always acted so childish because was never a child when he was supposed to be one. and now this great responsibility falls on his shoulders, the kamikake. giving himself to an ancient god in order to safe soul society, something he always knew was going to happen eventually.
