Please Help

I have a question to all of you that have read Tokyo Ghoul. I have not read it all myself, but I have watched the anime and loved it. Now that tg:re is coming out, I really want to read it, but I still don't understand one important thing: what happened to Kaneki?? I've heard he wen't crazy because of Rise and the torturing, but can anyone explain exactly what caused his 'death?'
