Note: I am only including those who are explicitly presented as trios, not just movies with 3 Kelly (age) characters, so no Nutcracker or Swan Lake.
This is truly a shot in the dark.
I vividly remember a childhood friend in the late 90s/early 00s having a Kelly (?) circus set. In my memory it had a circus train with three cars that were open air pink plastic with working wheels, and an animal. It was too small to have been for Barbie and had a smaller doll with it who I vaguely remember as being blond in a circus-performer outfit.
My friend did have an older sister so it may have been from an earlier era.
I have scoured the internet and cannot find any circus or carnival Barbie that matches my memory. Do you (or your followers) have any idea if this was a real toy I always wanted to play with or something I’ve made up?
You sent this a while ago and I didn't reply at the time because I was experiencing a chronic bout of can't be motivated -- but I have been thinking about this on and off since I first saw it and thought I'd put a bit of thought into this one.
There have been some Kelly Club circus-adjacent sets, like Kelly and her friends in various circus/carnival outfits.
But no circus themed playset that I am aware of.
I do want to draw attention to this Nursery School playset from 1997, though.
Apart from the fact that it isn't circus themed, this fits your description fairly well. Pink train with open air carriages, sized for a Kelly doll, from the late 1990s to early 2000s. No animal, and no blonde circus performer though.
But is it possible that this was the train, and the animal and circus performer doll were from different releases and you and your friend just put them together to turn this nursery school train into a circus train?
It's also possible that whatever this was wasn't a Barbie release -- there's a lot of other fashion dolls and kid dolls on the market in other brands that sometimes confuse our memories. If it turns out this train isn't what you remember, that's my best guess.

I've been surprised before by Barbie products where I expected to see maybe one or two and found dozens - laundries, for example. But another on these lines where I am simply surprised by the number of variations on a theme that I wouldn't have been at all surprised for Mattel to have never explored... is the humble bathroom.
I understand the appeal of Barbie, but I struggle a little more with Barbie having a bathtub and toilet. It has never occurred to me that for some young ones, making Barbie take a shit is an integral part of the gameplay.
There's been toilets involved in some sets specifically themed around children. I'm thinking of Potty Training Kelly or the potty training playset from the Skipper Babysitters, Inc series. The idea being that you are assisting with a child learning to use the toilet in the first place.
But Barbie herself sometimes also needs a restroom. And, in fact, not just any restroom, but one to match the Barbie aesthetic. My favourite example of this specifically is Glam Bathroom from 2013.
Does Barbie need her bathroom to be glam? Damn right she does.
Another playset from 2007 has a much humbler name (Barbie Bathtub & Toilet Bathroom Playset) but also features a design that looks right at home in Barbieland in a way that no toilet and tub feel like they should.
It also features a cat climbing out of the toilet. I have heard that people with curious pets should keep the lid down to avoid that. My toilet, like Barbie's, actually doesn't have a lid, but my cat has only drunk out of the toilet once and it was when the water in her water bowl was too low and she was making a statement.
It is also important for me to tell you that the Barbie camper vans have a toilet in the back.
I would never have noticed its absence, but it was covered for me.
On the topic of unsanitary I used to have a playset with a Barbie and a Kelly doll and some kind of tiny strawberry kitchen. It came with a sweet powder you’d mix with water in the kitchen but I was way too young to do this on my own unsupervised.
Didn’t clean any of the containers before mixing whatever it was supposed to be in it, probably scooped it out with my fingers or even licked it out of from the toy when it was done(?). The little kitchen and dolls were sticky for years afterwards until my parents threw them out when I was older

This definitely falls into the same category to me of why would you market this to children. They are going to lick it and make a sticky mess. They are going to put food in a dirty toy and then eat it. I mean, children should eat dirt to develop their immune systems I guess but… sticky?? Ants??
Do you know something about scented Kelly dolls that came with a movie? They go to candy land or something?
I think you might be thinking of Sweetsville?
Although Sweetsville wasn't exactly a movie, as the video was only 22 minutes I believe. There might be another release that I just can't think of right now though.
What's extra notable to me though is that when you look up Sweetsville you get results for what to me are "normal" looking Kelly dolls but also these unsettling off-model dolls.
Their faces are more oval. They're described as a "cute new look" but they make me feel like I've stepped into a parallel universe where everything is just slightly different in a way which slowly makes me increasingly vulnerable to the beings that created the alternate dimension.
Top of the collection to me though is Duke de Sucre because I think Duke de Sucre is a truly excellent name for the aristocracy of a candy world.
I hope this was the set you meant because otherwise I just talked for a long time about the wrong thing. But I hope it was interesting at least!
