Combining lasers & volume replacement can give good skin rejuvenation results . . 🔬Skin Science: Ablative lasers including erbium & #co2lasers target water in the skin. They can be employed in fractional mode, or fully ablative. For wrinkles like these, I prefer ablative lasers, one treatment only. Though lasers can improve collagen, they do not give a significant increase in volume, unlike dermal fillers . . 🔫Lasers & downtime: The deeper the laser, the higher the coverage the longer the downtime. This ranges from 5 days to 15 days. Additionally redness can be persistent for up to 3-6 months. This can be treated with vascular lasers . . 💉Filler: I used 1.5 ml Juvederm each side. 0.5 to the piriform fossa. This point blunts the NL fold, supports the medial cheek, & in some cases helps with the upper lip. Low volume, max gain. 1.0 ml used to build the orbital rim. Ablative lasers are still relevant in dermatology - especially in the mx of some forms of #acnescars. This patient was treated with deep peels 🍌🍌followed by one session of fully ablative CO2- old school method. Downtime 2 weeks📆 . . 🔬Skin Science: Ablative lasers include erbium and CO2. These lasers can be delivered in fractional format (fractions of skin treated ranging from 1-70+%) as well as fully ablative mode (100% of skin treated). The target is water. In this case I raised the scars up with TCA & old fashioned phenol croton oil peels (applied focally) prior to ablative laser resurfacing . . 💯Indications for lasers and peels: Superficial acne scars, namely box car scars, pick scars, mild atrophic superficial scars. Deep peels & lasers are also great for wrinkles- see photo . . 🔫Ablative lasers I use: Erbium Sciton Joule (fully ablative - fractional), CO2 by Lumenis Ultrapulse, CO2 by Syneron Candela CORE, CO2 by Lasing USA, #Mixto, CO2 by Lutronic eCO2 (shortly, hopefully) . . 👍🏻Why I like CO2 lasers: Because if you need to do the job correctly the first time around, CO2 lasers deliver. I use different pulse durations & focal lengths based upon the job. No proprietary DrL method (LOL), just old school CO2 resurfacing.
