Okay, let’s take a look at the radio mail form first:

Now we will break this into parts so you can understand what everything means (pictures are above the explanation):
As you can see, in the “Category” scroll down there’s only Tayoriniaru radio, so you leave it as it is, of course
> “Radio name” – here you will write a radio name/ pen name/ nickname. It can be anything, and it doesn’t have to be something you will always use for radio mails or for mails for Ore Monogatari radio. It will not be saved in any computer memory/mail form memory, and you can choose another nickname each time you send an e-mail.
> “Mail address”- your mail address, of course.
> “Prefecture” – if you’re living in Japan, you pick one of the prefectures written there, but if you’re reading this guide, then I bet that what you will want to pick is the very last option, which means “overseas”.
> “Gender” – pick between the two, the upper one is “male” and the bottom one is “female”
> “Age” – here you write your age; usually for this kind of thing there were categories like “between 24-30” or such but it seems they want you to write your actual age here.
> “Corners” – pick a corner that fits the content of your mail; if it fits in one of the first two categories, select those, but if it does not, just pick “usual me”. More info on corners here. (page will be added at a later time, check ep 01 summary for a quick introduction)
> “Mail content” – you should write your mail here; whether you chose to send it in English, asked someone to help you translate, sent me a request or used google translate although I don’t encourage you, here is where your mail goes.
Then click on the bottom blue button that means “confirm”
After you click “confirm”, you will be shown your message again, with all the info you entered. Check everything again, then click “go back”(first button) if you want to go back and edit something (you won’t need to write everything again, everything will still be input; don’t click on backspace to go back because your message will be lost), and click “send”(second button) if everything is ok and you wish to send your mail.
By the way, I am not sure if it’s obligatory to write every information they ask for. I didn’t try to send a mail, but maybe it will still get sent even if you don’t write your age and such.
