Sasuke's perspective right after Itachi's death being drenched in dissociated body horror…..God. It's so visceral. He's not just surprised to be alive, he's disgusted. We already knew sasuke is at the very least passively suicidal for most/all of shippuden, but like. Yeah. He really thought the fight would end one of two ways: they kill each other or itachi kills him. He was fully confident he would die there, just thought maybe he was strong enough now to take his brother with him. The repeated emphasis from Sasuke's perspective of both the rain and the black fire continuing to burn. Points out a contradiction in nature (on top of the already existing contradiction of black fire) that feels a little unsettling and surreal. You know. Cause Sasuke is literally in shock and probably about to throw up. Focusing on how surreal the landscape is because the moment itself is surreal. Wrong. Itachi is dead and Sasuke ISN'T.
