I guess it would kind of be post-apocalyptic in a way, about two hundred years later. The setting would be in one of few cities that were salvaged and re-built, although there are a lot of labyrinths and subterranean tunnels made by those trying to survive the initial apocalyptic event. There's a new disease going around, something researchers have no idea how to combat (going with a virus or prion here but virus seems like my best bet). Humanity would be left with a weakened immune system (or just kidneys?) leaving many with what's sort of like a dialysis machine attached to them, constantly filtering their blood and pumping in white blood cells. There would be a group of scientists-turned government officials. They decided a long time ago it'd be easier just to eliminate those with the disease than to cure them, for various reasons. The main character I think would have lost either their family or their significant other to the disease. The scientists are hunting them down because there's a good chance they have it too. As soon as the symptoms start to kick in, they go into hiding in the tunnels under the city that lead to the badlands outside. They get taken in by a team of rogue scientists who are trying to find a cure but the base is eventually destroyed and they're taken in by authorities.
