Unlike ink, they respond very quickly to q switch or pico lasers; the latter is a bit of an overkill. Smaller particles are ‘blown up’ with photo-mechanical-acoustic lasers & are removed by the immune system. Much bigger particles can be treated with simple microneedling over fractional ablative lasers. I had to think outside the box for this case of scar revision . . 🔬Skin Science: One of the most challenging cases of scar revision. Scars are highly atrophic with elastolysis & outpocketing due to lack of support. Additionally there was significant subdermal fibrosis involving the neck & face. Adjust your settings on the run, based upon depth of correction . . 🔬Skin Science: To date, the vast vast majority of scar revisions are based upon SOP or standard operating protocols. I get it, you can not write a scientific paper based upon variables such as- ‘in this patient I did 2.2 mm RFM, 45 mj 2 passes then I went to 1.6 mm then used an erbium laser at 300 microns here but did 1200 microns on this area.‘ It won’t fly in any journal... . . 👍🏻👉Reality: Scars will vary in morphology from patient to patient, and even in the SAME patient they will vary even in the same location. Try placing your cannula in the dermis- subcutaneous junction, then ‘lifting’. In some cases you can actually SEE the dermal atrophy. Dermal thickness varies between patients & region . . 🔫Dialling in the artillery: Before RFM or laser, think before you pulse. Dial in the settings based upon the scar you are treating. This provides higher accuracy AND less collateral damage. Devices are only tools, pick them according to the job required. The relationship of age related volume loss (or physiological lack of volume) & acne scarring is one of the most challenging concepts to get across to patients. I have this discussion at least twice a day! . . 🔬Skin Science: Acne scars can be classified as hypertrophic (raised), or atrophic (divot). The latter can be in the dermis, the subcutaneous layer or both. Primarily dermal atrophy is very easy to correct, in the majority of cases. Deeper fat atrophy is harder. The use of dermal fillers or fat transfer is required in some cases. Proper #subcision techniques together with the art and skill of dermal filling are the most important skill sets to master in the context of scar revision. Laser & energy devices can be mastered in a relatively short period of time . . 👉🙄Age related volume loss: Support for the OVERLYING scars deplete with age. By looking beyond #acnescars and providing volumetric filling can result in traction & support for scars. The area of volume replacement will depend on the presentation. My usual 3 point areas include the zygoma, ramus of mandible, & in the area of the mandibular cutaneous ligament, modified accordingly 💯👍🏻 . . 👀Practical aspects: Treat the pattern of acne scars with appropriate methods. Address any volume deficits, enhance aesthetic. Really challenging scar type; broad based boxcar scars on the forehead, background atrophy . . 👀💉What filler to use? This depends on many factors including location, age & clinical examination. HA fillers are great for fine work, whilst #collagen stimulating fillers such as PLLA, PMMAs or CaOH apatite can be used for larger volume correction . 👍🏻💉In this case I used erbium over CO2 laser as I want product penetration. Simple derma roller was used over #microneedling as I had the need for speed. Pick up an instrument for the job, and not limit your choices. Before and afters will be posted once the job is complete.
