Random South Park headcanon:

I think it's fair to assume that most South Park characters look like normal humans in-universe, rather than paper cut-out cartoon characters, based on various character's comments on eye colors, noses, and the like. However, the Canadians obviously look visibly different from most characters, hence Kyle hiding Ike in the movie out of worry for his safety and Sheila's reference to their "beady eyes" and "flapping heads". Thus, I believe that Canadians in the South Park universe share various features and traits which make them distinctive, such as:

-small eyes with large, dark irises

-small noses

-wide mouths with thin lips

-round faces

-blocky bodies with square shoulders and flat chests

-a habit of opening their mouths wide while speaking

Thus, while still obviously human, the Canadians are also easily recognized instead of in the real world where they look pretty much like everyone else, which could have a large impact on the differing relationships between South Park Canada and the rest of the world versus actual Canada and the rest of the world.
