To remove systemic discrimination
Recognise and remove prejudice, see a person beyond their appearance we initially perceive. Judge people by their actions, with consideration of their understanding of these actions.
Analyse causes of inequality (e.g. policies that statistically benefit people from certain backgrounds but not others) and remove the relevant barriers, if removal is not possible then compensate those disadvantaged equitably. Promote inclusion, respect, and equality. Raise awareness of diversity.
Listen to the voices from communities without being driven by populism (term as according to http://www.bbc.com/news/world-43301423) - the "them" and "us" ideology fuel divide over communication and cohesion. Compare what people are saying with available statistics. Consider new statistics / attributes to collect. Create goals that are measurable whenever possible. Plan, Do, Study, Act.
Finally, accept that many forms of systemic discrimination can be minimised, but not eliminated, unless there are no barriers (such as legal fees). We can keep fine-tuning or revolutionising systems that have been put in place, but do note discrimination mostly stems from an individual's ignorance, stereotypy, desire, and greed.
