It's not literal (it's usually not) 🫣 I have this on my ascendant 22° so I can understand why you're saying this.
I'm going to be honest with you, life might have not felt fair to you because of how much you had to go through. This degree adds a serious karmic intensity to your energy and it really depends on where it is in your chart. For you, it means transformation can feel very painful and very serious. But it doesn't necessarily mean literally painful. We tend to go through so many internal, spiritual, and emotional transformations that we never speak out loud, and with this place specifically, it is your soul's mission to embrace change through the hardest times rather than cleaning on to what you know out of fear.
This degree represents a metaphorical and spiritual death and rebirth cycle that you will go through in your life, likely more than others. It also depends on what your Pluto is in as Pluto at 22° in Scorpio is different than Pluto in 22° Libra or sagittarius.
What is more curious to me is what's going on with your Pluto sign, if you're making any major aspects with any big planets, etc. I feel like the aspects and other placements like your mars, your Saturn, 6th, etc to tell you more about your "luck", health, agility, etc.
I've noticed that this degree just makes life and that specific area of energy according to the planet, a lot more intense than others might experience. For me personally as it's on my ascendant, I do kind of feel like I've had a little more dramatic life experiences than most so it's made me a bit more of an intense person lol but the downside to that really is that I can see in black and white sometimes. That could be an issue that you might feel too as it is a 22° issue. But don't worry about anything happening, I don't love how this degree is so fear-mongered. Astrology can't determine the time of death anyways.
