Hump removal makes such a difference to the overall look of a patient. Jumps tend to make young female patients look older than they are. The drooping tip is a common reason for patients to have rhinoplasty. Here is a recent case of a teenager who needed her tip elevated and the tip made less bulbous. This was done using septal cartilage and by trimming the end of her septum to shorten the nose. Not pictured here was also the fact that the nose was very crooked and required significant nasal airway reconstruction. The main thing for me to make sure is that there is good central and lateral support to the tip. This primary rhinoplasty immediate on table result was achieved with open structure rhinoplasty. There were three separate approaches I considered for this patient’s nose. Her problems included: dorsal hump, weak, drooping tip, left deviated caudal septum visible in her left nostril, and a high right septal devotion. Approaches could have been open rhinoplasty, closed rhinoplasty, or preservation rhinoplasty. I ultimately decided on open rhinoplasty because of the septal deviations and the very weak drooping tip. This topic will be discussed on a panel at the upcoming Rhinoplasty Society meeting in Las Vegas. As President of the Rhinoplasty Society, I am very interested in making sure we have the best educational content possible. Subtle changes of the bridge and tip of the nose are always a possible. Rhinoplasty does NOT have to be extreme or overly stylized.
