Fousey update: after his incredible trip to Africa, @fousey came in to continue the project of nose reconstruction. This is key because if you remember, he did not have “rhinoplasty,” he had nasal septal fracture reconstruction. Basically, his nose was mulch after a direct hit to the middle of his nose during a boxing match. His entire septum needed to be rebuilt as did the middle vault. I used rib cartilage and restructured his bones. The tip needed to be supported, as well. He had a saddle nose at the time we did the rib graft. Now, we have to balance the soft tissue and make the nose look like nothing happened. This will take some work, but he should not need any more operations. More to come... First laceration patient to avoid the Emergency Department with an in office repair. No waiting. Very nice plastic surgery repair of a nose laceration - and off we go! This is the immediate on table result after rib graft reconstruction of a severe nasal septal orbital fracture.Remember this guy? He unintentionally launched his car 10 feet off the ground through a telephone pole a few months back and casused the car to look like it does on the third photo. We performed a closed reduction of his nasal fracture at that time. You can see the Instagram post back in August and the preop nose in this post. He will do very well from here out. However, there is still a 10% chance of RE-operation of some sort. Nasal septal trauma and revision rhinoplasty (and primary rhinoplasty for that matter) all present the issue of further revision surgery. Obviously the more complex the operation and the situation, the greater the chance of reoperation. @fousey did a great job raising money for children in a celebrity boxing match last weekend. Unfortunately, as in all boxing matches, he took a few to the nose and now we have to fix him up. The approach for a nasal septal fracture like this is to first do a closed reduction of the fracture and put the bones and the cartilage back as best we can without actual open surgery. The reason we cannot open the nose is that the tissues would just simply fall apart and it would not turn out right. Then, in 4-6 months, we will do a formal open treatment of the broken nose and septum, a true Rhinoplasty but for the fracture repair in addition to the aesthetic issues (size and shape). I appreciate @fousey for sharing his story as facial trauma is common and man many times it is mishandled. And he is a true champion raising money for children of war. 3 week post-op rib cartilage graft rhinoplasty for nasal reconstruction. This patient had a fractured nose and waited 4 months for her Rhinoplasty / late fracture repair. She has some residual edema but is acceptable for going out to dinner, some photo ops, etc. it will get better and better, but it is important to understand the post-operative healing course. Today we took care of a young man who suffered a telescoping nasal. His nose was pushed posteriorly along his skull base resulting in a saddle nose and super-deep radix. You can see this on his CT scan on the second photo. Our team was happy to lend a hand today with my top-not h assistants @drnvplasticsurgery @dr.erik.wolfswinkel @roveloplasticsurgery @josevasquezla. Very pleased that the bone reduction ruins were successful. The septum, however, will need to be rebuilt at a later date.
