Imaging AI Reimbursement in the U.S.

The road from FDA clearance to real-world adoption is long — but it’s absolutely possible. A growing number of imaging AI tools are now being reimbursed in the U.S., either nationally or through regional MACs. To get reimbursed by Medicare, an AI tool must first have FDA clearance, and then follow one of a few possible payment paths: 🔹 National coverage usually requires a permanent CPT code (Category I), or occasionally a HCPCS code when the tool is bundled with a device or service. 🔹 Regional coverage through Medicare Administrative Contractors (MACs) can happen even without a dedicated CPT code, as long as there’s strong clinical evidence and a clear medical use case. 🔹 In hospitals, some tools may also qualify for payments through CMS’s NTAP program, which helps cover new technologies during early adoption.
