[[ Ignoring his death in canon, but retaining his illness, Itachi will only live to be about 40-45  years old. 

How he will age into and old man though-- It will be a graceful aging. Itachi will not attempt to keep up his youth or appearance. When his hair begins to turn white, he does nothing to stop it. It thins out really bad, too, and is nearly translucent, losing its previous thickness.

He becomes quite thin, bones showing in his wrists and ankles, and knees, and bruises very easily. His skin sags the most around his eyes and elbows. He will eventually be too weak to walk, requiring a wheelchair.
