d'you think napstablook knows that mettaton is their cousin? i just did the true pacifist ending again, and blooky says that they're "a huge fan" to mtt. is there any other dialogue that would suggest they do/don't know?

There is really not enough evidence to support that Napstablook knows Mettaton is their cousin. Napstablook never mentions their cousin, and they only speak to Mettaton as a fan – not as family.
NAPSTABLOOK:oh……..hi… mettaton…i really liked watching your show…my life is pretty boring… but…seeing you on the screen… brought excitement to my life… vicariouslyi can’t tell, but… i guess this is the last episode…?i’ll miss you… mettaton………oh…. i didn’t mean to talk so long…oh……….

The fact that Mettaton is a ghost that became corporeal appears to be a secret kept from his fans, and possibly even from Napstablook. Instead, the story is that Alphys created Mettaton as a “human eradication robot.”
METTATON:AS ANY TRUE FAN WOULD KNOW, I WAS FIRST CREATED AS A HUMAN ERADICATION ROBOT.IT WAS ONLY AFTER BECOMING A STAR THAT I WAS GIVEN A MORE… PHOTOGENIC BODY.

On the other hand, Napstablook never mentions their cousin, and no one knows what happened to Mettaton.

Except for Alphys of course, who created Mettaton’s robot body. All this is revealed in Mettaton’s diaries and mentioned in one of Alphys’ true lab entry. Perhaps, Alphys and Mettaton are the only ones who know that Mettaton was given a robot body to become an entertainer. However, Napstablook prefers to not be rude and stay out of other people’s way.

It’s possible that Napstablook knows that Mettaton is their cousin and chooses to stay out of his way to stardom. After all, Mettaton’s “dreams can’t wait for anyone.” Perhaps Napstablook read his diary, or maybe Mettaton did tell Napstablook – and they are just too polite to quit the charade. Ultimately, there isn’t enough evidence to know. It’s up to interpretation.
