In the past, I have also had students use the worksheet to present themselves in front of the class. I also encourage teachers to fill out the same sheet so students can find out about their teachers too! 

This is not just handy for back to school but also for themes on identity. 

There’s a black and white version so students can add colour and a colour version for a quicker activity.
