I think the funniest context and outcome to this scene is if Kayn was legitimately diagnosed with a form of schizophrenia at a young age whilst still in Noxus. So Zed and the rest of the Shadow Order---most likely having this knowledge---just think he's losing his shit whenever he says things like this.
The mystery only gets solved once he forgets Rhaast on a coffee table somewhere, eventually leaving someone to pick him up and implode into Darkin hemo-matter.
