When nsg writes “Disoriented to place, time, situation” on my patient with expressive aphasia
...and I know for a fact the Pt is FULLY oriented, but cannot express that information due to a little thing called EXPRESSIVE APHASIA.....
*sigh* Time for a little staff education!
Entering a pt’s room for tx and suddenly remembering they’re on c-diff precautions like:
BEWARE THE DIFF
When the RN/CNA reports the patient's teeth were brushed this morning...
...but then I spend an entire 60 minute session wielding a suction brush and scraping copious secretions from the patient's mouth...
