Since "The world is cruel but I still love you" is a direct twist on Mikasa's famous phrase, I'd say we're supposed to assume it's primarily directed to her, but that doesn't mean it excludes his friends.

I'd say the sentiment is similar to the one in the train car in chapter 108. Eren was speaking generally to everyone there, but Mikasa was given special focus.
