February 28, 1966 — Astronauts Elliott See and Charles Bassett are killed in a plane crash

See and Bassett were scheduled to spend three days in orbit in May 1966, during which they would dock with the Gemini-Agena Target Vehicle. Bassett would perform an EVA. However, they were attempting to land in inclement weather in St. Louis, Missouri when their T-38 Talon struck the roof of the McDonnell Aircraft building where their spacecraft was located.

January 16, 2003 — STS-107 Columbia launches

STS-107 was the 113th flight of the Space Shuttle program and the final flight of Space Shuttle Columbia. During its nearly 16-day mission, it carried the SPACEHAB Research Double Module, the Freestar experiment, and the Extended Duration Orbiter pallet. Mission Specialist 2 Kalpana Chawla was the first woman of Indian descent to fly in space while Payload Specialist 1 Ilan Ramon was the first Israeli astronaut. All seven crew members were lost when Columbia broke apart during reentry on February 1.

February 21, 1969 — The first N1 moon rocket launch ends in failure N1 3L, the first of four launch attempts of the Soviet N1, was to insert a prototype lunar spacecraft into lunar orbit. At T +6 seconds, pogo oscillation in one of the first stage engines triggered a propellant leak and a fire erupted. At T +69 seconds, the KORD, the rocket's engine control system, shut down the first stage and the vehicle fell back to Earth.
