2000AD Programme 1, dated 26 February 1977. Below, from Collector's Gazette No. 470 (May 2023), an issue sold at auction for £160 (with Space Spinner) in March 2023. Rebellion | The Dan Dare Corporation.
The Wizard No. 102 (22 January 1972) and 162 (17 March 1973). Either they had a lack of new ideas for free gifts at this point or they had ordered huge amounts of these. Probably a bit of both. Can't remember the same free gift being given away so soon after the last one though.
It probably reappeared many times after this as well. In September 1982 The Victor gave away the very similar looking Super Whizzo!

1983 ad for the "new-look" Eagle. New-look often meant that it was printed on cheaper paper.
A grey/silver Space Spinner had also been given away with the first issue of this new Eagle in March 1982. This one was red, of a different design and came sellotaped to the cover of the issue dated 15 October 1983. Good luck getting it off and keeping the comic in mint condition.
