Cas, while under Rowena’s attack dog spell in 11x3, beat Dean to a bloody pullp, and he feels so guilty about it. As far as I can remember, the only other time Cas ever hurt Dean even close to that degree was when he was under Naomi’s control (the crypt fight) and the fight they had in the alley in 5x18 over Dean intending to say yes to Michael.
Just to quickly address that fight in the alley: I don't think it was all Cas, not the Cas we now know. It was his angel programming, his wrath, his righteous anger. Rather than Cas, I feel like that was Castiel. New to rebellion, new to friendships and emotional connections, new to betrayal. Let's be real, Cas had the emotional maturity of a toddler at this point. I honestly don't think he knew how to handle Dean in that state other than violence. The friendship they had was still new at that point despite Cas already having given everything for Dean, hence his frustration. I don't think he knew how to cope or deal with his feelings about what Dean had been planning to do. In the end, he did manage to rein in his anger, however.
Back to my main point, though: Even when Cas does have to fight Dean, he does his best to subdue him, not hurt him.

Cas does this every time. He doesn't want to hurt Dean. So, even when he has virtually no control over his actions (the attack dog spell), he feels especially guilty. Once things have settled down and they're home safe in the bunker, Cas reaches over to heal him—as he typically does. This time, though Dean declines.
Then, this happens:

They have a whole silent conversation with just these glances. Dean does say he had it coming, and we all know he's referring to the fight in 10x22. A busted lip, a black eye, and a swollen cheek is his penance.
He looks down as he tells Cas he'll just deal with it, but he looks up to meet Cas' eyes for a moment afterwards (the middle gif of the three above). This is why I'll never get over this scene. We all know Dean isn't good with words or apologizing or expressing his emotions. That look is just... monumental. It says, I'm sorry I did that to you. I care about you, and I hate myself for hurting you. Healing the old fashioned way is my penance, as little as it is. I don't hold any of what Rowena's spell made you do against you. I deserve this.
Cas knows Dean well enough to understand that. He responds with the look in the third gif. He tilts his head just a fraction, his mouth forms a slight frown, and his eyes say, No, you don't deserve this. I know you didn't mean to hurt me, it was the mark. I forgive you, and I know you forgive me. You don't need to hurt for me, but I know you won't let me help you.
And that's just it, y'know? It's just them. Their whole romance is lived in looks and instances like this. They share these silent conversations, these moments of understanding. They do this not because their feelings are something trivial or simply not there, but because their lives are tumultuous and dangerous and chaotic and they both have a hard time putting themselves and their feelings front and center when the world could fall apart around them at any second. They can't even process their emotions half the time, much less talk about them. Moments like this are blink-and-you'll-miss-it, but holy hell—they're everything.
Dean and Cas? They fall in love in glances and moments of silence.
I'll never get over how touchy Dean is with Cas in 11x3. The whole attack dog spell mini-arc was an excuse to have Dean touch Cas as much as possible tbh in the bunker when Dean would not let Cas go when he fell to the floor and then he wrapped the blanket around him??? the warehouse when Rowena removes the spell and he falls again??? The way he cradles Cas' face???? So tender.

You know why they gave Jensen this episode right? And promoted the hell out of the fact that Jensen is directing this episode. So many watched for Jensen because excuse me, he is fucking fantastic and cinematography of bad seed was amazing.
But no one was pulling off this episode and the crappy writing in anyway that would have made sense.I still can’t believe that demons have souls now.Seriously, what?
Its not like they don’t know how invested fans are in Jensen.So, hey lets give him the most ridiculous script to direct.
I am always gonna hold it against them.I wouldn’t have watched it if I wasn’t invested in studying every scene for all the technical reasons and all the details Jensen put in them or if J2 weren’t so enthusiastic about it.

I am not happy that the episode’s ratings tanked but what were they expecting? Hey, we have put everything you want here, Winchesters finally treat Cas like a friend not their personal heavenly encyclopedia and look they care so much,and hey some fanservice for the shippers we torture and starve so much and we even got Jensen to direct this.Yeah well, it didn’t do you any good.Better luck next time.
And oh god, modern family,criminal minds and even arrow had steady ratings.Empire saw an increase in ratings after 3 whole weeks.What else do they need to see that they got to sort their shit out and firstly maybe get all the writers on the same page and maybe also educate them on 10 years of supernatural.?

there aren’t words.
Dean shifts slowly in his bed, wincing as he tries to get comfortable. His jaw is still tender, and his ribs prickle uncomfortably when he breathes in.
He needs another beer, but knows somehow, once he rises to his feet, that the kitchen isn’t where he’s headed.
The door across from his is slightly ajar; he pushes it open easily, silently. Cas looks up from where he’s sitting at the foot of his untouched bed staring at his hands. From what Dean can tell, he hasn’t moved in the five hours since Dean led him there.
He looks surprised - it’s 2am - then not at all, as his eyes land on Dean’s injuries. He frowns and lifts a hand, two fingers outstretched, completely misreading Dean’s intentions.
Dean sighs, stepping forward and gently pushing the hand away, his own fingers wrapping around Cas’s wrist.
Cas’s eyebrows furrow. He opens his mouth to speak, but Dean shakes his head. There aren’t words. His hand slides from Cas’s wrist to his arm to his shoulder to his neck, where it stays.
Cas looks up at him, his mouth parted, his pupils blown. His hand comes up, tentative, to grip Dean’s waist. There aren’t words. There’s no need.
Two pairs of eyes close - one in penance, the other in disbelief - when Dean leans down to press his lips to Cas’s forehead.
When he pulls away, it’s to pull Cas closer, his arms wrapping around the angel’s back like wings. He feels Cas’s forehead pressed against his chest and Cas’s breath stuttering through the cotton of his T-shirt. Where his ribs ached five minutes ago is now a completely different kind of ache, one that begins to lessen when Cas sighs and runs his hands down Dean’s sides.
